Family Therapy at Maplewood Is Different

Most family therapy practices in New Jersey work with young children. We don’t. Our family work focuses on the harder stuff that generalist therapy isn’t built for — parents and teenagers locked in escalating conflict, blended families navigating loyalty bonds, adult children who’ve grown distant, sibling tension around aging parents, and family transitions that strain even strong relationships.

Our senior clinicians — LCSW and LMFT — hold the room when emotions run high. We’re trained in attachment-based work, family systems therapy, and expert-guided family work, which means we don’t just teach communication scripts. We help your family understand the patterns that drive the recurring fights — and what to actually do about them.

You don’t need to wait until the family is in crisis. Many of the families we work with come in when one thing is off — a teen pulling away, a stepfamily not gelling, adult siblings drifting — and they want senior expertise before it becomes something larger.

The Family Work We Do

Parents and teenagers (13-17)

Communication breakdown, mental health concerns, school refusal, identity exploration, family rules vs. autonomy. We hold space for both the teen’s voice and the parents’ very real worry.

Parents and older teens / young adults (17-25)

The launching years. College transitions, autonomy conflicts, money tension, what it means to stay close while growing apart. Includes work with families navigating mental health or substance concerns.

Parents and adult children (25+)

Estrangement, communication breakdowns, holiday tension, partner disapproval, grandchild access, role reversals. Adult family dynamics are different — and they need different work.

Blended and step-families

Loyalty bonds, step-parent dynamics, ex-partner co-parenting, “my kids vs. your kids” tension, building new family identity. We hold all the complexity without taking sides.

Adult siblings + aging parents

Caregiving conflict, inheritance and end-of-life conversations, sibling rivalry resurfacing in midlife, who-does-what dynamics. Sometimes the parent is in the room, sometimes the siblings come alone.

Family transitions

Death of a parent, divorce after long marriage, serious illness, family secrets coming out, identity disclosures (gender, sexuality, recovery). Major life changes need senior clinical support.

Open 7 days a week — including evenings and weekends. Family therapy works best when everyone can be in the room. We schedule around real family life, not 9-to-5.

Insurance & Fees

Out-of-network by design. Maplewood is an out-of-network practice. This is intentional: it lets us protect session length (60 and 90 minutes — not 45), keep caseloads small, and choose modalities based on what works rather than what insurance will pay for.

How out-of-network works for most families: we provide a superbill after each session for you to submit to your insurance. Most out-of-network plans reimburse 50-80% per session. We accept HSA, FSA, and HRA. We can help you understand your specific benefits during your free 15-minute Fit Call.

What age children do you work with in family therapy?

Our family therapy practice focuses on adolescents (13+) through adult children — not young children. Families with little kids typically need a child-specialty practice or play therapist. We come in when the family dynamics are complex enough to need senior clinical expertise: teenagers in crisis, blended-family tension, adult-child estrangement, sibling conflict, and major family transitions.

Does the whole family need to come to every session?

No. Family therapy is flexible — sometimes everyone is in the room, sometimes parents come without the teen, sometimes adult siblings come without the parent. We design the session structure around what will actually move the needle. Many families do a mix: full-family sessions for active work, parent-only sessions for strategy and processing.

What if my teenager doesn’t want to come?

This is common — and not a deal-breaker. We often start with the parents alone, working on the parental side of the dynamic. Teens frequently agree to join after a few sessions, once they see something is actually changing at home. We also offer individual sessions for the teen alongside family work when appropriate.

Can family therapy help with estrangement from adult children?

Yes, and it’s some of the work we’re most experienced in. Adult-child estrangement is rarely about one event — it’s usually layered patterns. We work with the parents to understand what may have led to the distance, what’s repairable, and how to approach the adult child in a way that has the best chance of being heard.

How long does family therapy take?

Most families see meaningful change in 10-20 sessions, though it depends on what’s being worked through. Acute crises (a teen in a mental health emergency, post-disclosure family rupture) may need weekly intensive work for several months. Long-pattern work (adult-child estrangement, aging-parent dynamics) often runs 6-12 months.

Do you do family therapy with stepfamilies / blended families?

Yes — this is one of our specialty areas. Blended families have unique dynamics: loyalty bonds, “your kids vs. mine,” ex-partner involvement, step-parent authority. Generic family therapy often misses these. Our clinicians hold the whole picture.

What approaches do you use in family therapy?

Primarily family systems therapy, attachment-based work, and expert-guided family work. These approaches focus on the patterns and dynamics — not just communication scripts. For families dealing with trauma or one member’s mental health crisis, we integrate trauma-informed and EFT (Emotionally Focused Therapy for families) approaches.

Can we do family therapy online if some family members live out of state?

Yes, with one caveat: each participant must be physically located in New Jersey at the time of the session. NJ licensure law requires this. We have many families where adult children fly home or coordinate from a NJ location for sessions. Telehealth works well for ongoing family work once you’re established with a clinician.

What about families with younger children who need help?

We refer to trusted child-specialty practices and play therapists when the family work centers on a child under 13. Our clinical focus is teens, adult children, and the family-system patterns that affect them. We’re happy to suggest the right referral if your family needs a different fit.

Is everything confidential?

Yes. All sessions are confidential under NJ licensure standards. The only exceptions are the standard mandated-reporting situations (imminent harm to self or others, abuse of a minor or vulnerable adult). In family therapy specifically, we also discuss confidentiality between family members — what gets shared in individual sub-sessions vs. brought back to the group — during your first appointment.
